Practice Policies

Appointments:

Please call (508) 423-3483 to schedule an appointment.

The standard appointment is 45 minutes in length.

Insurance:

Susan Schneiders accepts Blue Cross and Blue Shield insurances.  If you have an alternative insurance company, you can pay Susan directly and submit a bill to your insurance company for reimbursement.  Clients are responsible for knowing the extent and details of their mental health benefits and reimbursement policies.  Reimbursement rates vary.

Confidentiality: 

As a licenced mental health professional, Susan is mandated to maintain the confidentiality and privacy of clients.  Exceptions to your confidentiality are outlined in the "Notice of Privacy Practices" as it relates to HIPPA.  Insurance companies do require a diagnosis for reimbursement to occur. 

Rates:

• Initial Evaluation:     $200.00
• Individual/Family Therapy:   $150.00
• Sliding scale fee is available.  Please address this with Susan Schneiders during the initial phone interview.
• Exposure Treatment may need extended sessions and will be determined with the client in advance.

Cancellation Policy:

Susan Schneiders requires 24 hour notice for session cancellation so that she may have the opportunity to fill that slot with another client.  A cancellation fee will be charged for those cancellations under 24 hour notice.
